High School Diploma or an official duplicate issued by the candidate's high school or notarized copy.
Transcript of records with number of hours per each course, ECTS, grades (and key to grading system attached), GPA and number of semesters completed and syllabi to completed courses
Sworn translation of transcript of records into Polish
Completed and signed application (you can find it here)
Recognition/Legalization/apostille of the High School Diploma when necessary (it does not apply to Polish “Matura” or IB).
Sworn translation of a foreign High School Diploma into Polish or English
Validation (notarization) of the candidate’s High School Diploma, if it was obtained in non-EU country. The validation can be done in Poland within the first semester of studies. Legalization should be done in the country of origin before arriving to Poland.
Two printed passport-size photos should be sent by post together with all documents; one scanned photo in jpg format should be submitted by e-mail.
Additionally, non-EU candidates are required to submit (kopiuj 1)
Medical certificate stating that the condition of your health allows you to be a student.
Health and accident insurance (immediately after the arrival).
Valid visa or Temporary Residence Permit / Karta Pobytu (immediately after the arrival).
Nostrification (notarization) of the candidate’s high school diploma, if it was obtained in non-EU country. The nostrification can be done in Poland within the first semester of studies. Legalization should be done in the country of origin before arriving in Poland.
